Parent's enrolment form WORD version PDF version
This form should be used if you are a parent or carer enrolling your own children. Please note that the Word version is designed to be completed on your computer. It can be opened in any application that can work with doc files.

School's enrolment form
This form should be used if you are enrolling a child on behalf of a school. A school is defined as any organisation, other than the child's own parents/carers, educating children. Please note that this is a Word document and is designed to be completed on your computer. It can be opened in any application that can work with doc files.

Payment declaration form Word version PDF version
This form should be used whenever somebody other than a parent/carer is paying for the student's fees. It must always be used alongside the school enrolment form.
